Abstract
It is known that both Hypertension and Diabetes impairs the function of endothelium and fibrinolytic system. We suspected that in patients with both hypertension and diabetes the impairment will be more severe. High endothelin level is one of the marks of endothelium impairment. If the common treatment aimed only to reduce blood pressure and sugar can modify the impairment? Wether we need more specific treatment to improve the endothelium function? The questions are what we are to discuss. 20 patients with diabetes(DM group); 21 with essential hypertension(EH); 19 with both EH and DM(DH), The rest were 20 normal volunteers (N group). All patients were treated in clinic and well controlled clinically(the blood pressure <139/89 mmHg; the fasting blood sugar<180 mg/dl ). Blood samples were got at rest status to measure the endothelin levels and the PAI acivities. Endothelin levels and PAI activities in DM and EH and DH were higher than N group(P<0.01), In DH higher than EH and DM(P<0.05); There were no significant differences in EH and DM. It was suggested that hypertension combined with diabetes makes more severe impairment of endothelium and fibrinolytic system than any of the diseases alone. Although controlled well in blood pressure and sugar with common treatment, the impairtment were not well modified. It is necessary to give more specific treatment to improve the functions of endothium and fibrinolytic system of the patients.
