A 27-year-old male, diagnosed with portal vein aneurysm during his childhood, presented with exertional dyspnoea recently. Computed tomography (CT) angiogram (Fig. 1 ) and volume-rendering reconstruction image (Fig. 2 ) showed portal venous aneurysmal dilatation that resulted from left partial anomalous pulmonary venous return (PAPVR). He was treated conservatively.
