On page 1618, right column, second full paragraph, line 12,“ 5%-23%” should be “5%-9%.” In the fourth paragraph, line 17, “48%-75%” should be“ 48%-77%.” On page 1621, first full paragraph, line 8,“ 16%-55%” should be “16%-35%.” In the second paragraph, line 4, “13%” should be “15%,” and in line 11, “31%” should be “23%.” On page 1624, first full paragraph, line 5, “36%-51%” should be“ 55%-63%,” and in the last line of the third paragraph, the incidence of grade 3 or 4 anemia should be 1%.
